Patch Testing Protocol

Patch Testing is an integral step in the overall process of successfully applying any permanent cosmetic pigment. One pigment can show different results on different people after the healing process is complete. In order to make sure that a certain pigment is appropriate for a given client, an IIPC Technician applies the pigment to a small, inconspicuous portion of the client’s skin, usually somewhere on the scalp in a process called a Patch Test. After the tested area has healed, the actual color of the pigment in the skin will become visible. Patch Testing is the only way to determine how a specific color will heal in a client’s skin, and if the client is reactive to the pigment.

All Technicians at the International Institute of Permanent Cosmetics perform Color Patch Tests before performing a procedure. The Patch Tests are quick and easy and can be completed during the client’s primary consultation. In most cases, IIPC Technicians choose a subtle spot on the back of the scalp in which to place the test pigment implants – unless the client requests to have it done in an area that is easily visible to them. Since the procedure is quick and easy, the disposable Soft Tap Manual Applicator is an ideal tool with which to complete it.

Here are the steps IIPC Technicians take when performing a Color Patch Test:

1. Determine the skin undertone and appropriate colors for your client. Remember:

                        Pigment + Skin Undertone = Final Color Result.

2. Mix the same pigment colors that will be used on the client for their requested procedure.

2. Cleanse an inconspicuous area on the client’s scalp (usually in the back) with alcohol, Hibicleanse or another appropriate disinfectant.

3. Photograph the cleansed area of the scalp prior to inserting any pigment to take note of any irregularities.

4. Using a 3 or 5 prong sterile Soft Tap needle, make 6 or 7 implants into the client’s scalp, in a cluster, on the cleansed area.

5. Take a photo of the exact area in which the Patch Test was made.

6. Instruct the client on the proper after care to be performed on the procedure area.

7. On the client’s chart, note the exact location of the Patch Test and  chart the color formula that was used. Include a photo showing the exact Patch Test area as well.

Any pigment patch testing requires a minimum 4-week observation period to check for allergies. Allergies may also appear after this time but are uncommon.

The administration of a Patch Test to a client does not ensure they will not have a reaction to any future pigments. Should the client experience any sort of reaction to the Patch Test during the week following the procedure, they are advised to contact both the International Institute of Permanent Cosmetics clinic and their personal dermatologist.

Me, a ‘Metro-Sexual’ Male ? Well, evidently so. In this age of vanity I, too, have
succumbed to the new age of appearance awareness. Meaning: to look good at any age.
Not since Ponce de Leon has anyone searched so diligently for the Fountain of
Youth. This century, by far, people are so appearance conscious that sales of products related
to aging are in multiple billions of dollars globally. Unlike de Leon, I may not have
found the Fountain of Youth, but I believe I have found an anti-aging procedure, one that
successfully removed 10 years of aging from my face. It is Skin Needling.
After a few procedures my face has a younger appearance. Ten years worth. The
deep wrinkles and laugh lines have diminished to such a degree that my adult children
compared pictures of me from the early 1990’s to my appearance today and are amazed.

Skin Needling and Scar Relaxation

Scar Relaxation was introduced by Susan Church at the first Society of Permanent Cosmetic Professionals (SPCP) conference in 1991 after she discovered it while working on several burn survivors in concert with Dr. Grossman. As you will see, this is still a relatively new procedure even though our clinic has been utilizing these techniques for over 13+ years.

We have lectured numerous times on this subject and have had articles and photos featured in several publications over the past 13+ years. These include lecturing at the Phoenix Burn Foundation National Burn Congress, Alicia Roush Burn Foundation, Surviving Burns Support Services, and articles for Burn Survivors Throughout The World, Inc. and The American Society of Esthetic Medicine Inc. You can also check out our chapter in the book “Aesthetic Facial Restoration” by the world famous plastic burn surgeon Dr. Elliott H. Rose of Park Ave in New York. This book is written for plastic surgeons so they may be able to ‘utilized three-dimensional subtraction analysis to fashion vascularized bone grafts for microsurgical jaw and orbital rim reconstruction’.

Both Needling and Scar Relaxation is a specific well-designed application to help create supple skin texture.

We consider both of these procedures in the category of Corrective Pigment Camouflage also known as C.P.C.

Needling, also known as ‘Skin Needling’, is accomplished by the technician utilizing a certain technique while inserting the needle into the top layer of the epidermis. This technique is best accomplished when it is used quite shallow in the skin.

Needling may reactivate the melanocytes in the skin. (See photos). We worked on an Africa-American and with beautiful results. His skin repigmented where we actually performed the Needling procedure.

Scar Relaxation is a different as the technician needs to work in the dermal layer of tissue for lasting results.

The technique for both Needling and Scar Relaxation is the application of repetitive circular movements called ‘obovoids’.

Scar Relaxation is used primarily to soften the fibrous bands of collagen (scar tissue) to give the patient more flexibility in their movements. (see photos) This procedure works well on all contracted tissue with exceptional results on the hands.

Needling is primarily used to ‘plump’ up the skin. This is the body’s natural response to an inflammatory tissue injury.

Scar Relaxation and Skin Needling appointments are usually scheduled at 4-week intervals. Dramatic results can be realized after the first treatment.

Patients are excited with their results of the C.P.C. that has been performed on them, many times alleviating the need for potential surgeries on the contractured tissue.

This excerpt is from an article in the Society of Permanent Cosmetic Professionals (SPCP) Newsletter in 1996.

“In all post procedure phases of C.P.C. there is an inflammatory response of the skins tissue. The skins typical symptoms may range from mild redness, heat, swelling and discomfort to lymph drainage. All patients will experience a typical wheal and flare reaction (redness, histamine release and swelling) to the procedure area that is much like a bee sting or mosquito bite. Applications of C.P.C. will also cause increased blood flow to the immediate area. The procedure area will be a bit sore, swell slightly to moderately within the first 24 hours, may be very tender and be slightly pink to red.

Immediate after care instructions includes the use of either petrolatum or some type of an antibiotic ointment. If possible, cover the area to keep it exempt from free radicals and pollution for 12-24 hours. If the procedure area is swollen, you may use ice for the first 24-72 hours to help alleviate the pain and swelling. Using heat after this period will promote circulation and removal of waste products. Injured tissue will heal much faster and scar less if the patient keeps the area moist. The patient should apply a thin coating of petrolatum over the procedure area while showering or bathing to protect it from moisture.

Patients should never use their fingers to apply ointments or petrolatum to the procedural area. All products should be applied with a cotton swab or tissue.

Technicians need to be cautious of post-inflammatory hyperpigmentation (darkening of the tissue) when applying C.P.C. Hyperpigmentation occurs when the skin’s tissue is traumatized. Hyperpigmentation occurs in Caucasian skin occasionally, although it will most likely occur on clients with heavy concentrations of melanin in their skin. Normal skin colour should return in several months following the application of C.P.C. Bleaching agents such as Hydroquinone, Kojic Acid and/or Licorice applied 1-2 times a day may be used to lighten the skin if needed”.

Skin Needling - Natural Collagen Renewal

What is Skin Needling Ms. Church has been performing Skin Needling for over 16 years. Her articles were published in 1991 in the ‘Society of Permanent Cosmetic Professionals’ (SPCP) Newsletters as well as in other publications on these remarkable findings. Ms. Church has also lectured to medical and beauty professionals on numerous occasions sharing photos of burn, scar, cancer, as well as ‘everyday’ clients that request her services.

Skin Needling/ Facial Needling treats light scaring, fine to deep wrinkles. Using a machine for the Skin Needling procedure, the needle cluster is gently inserted into the epidermis and top layer of the dermis. Skin Needling promotes the natural reproduction of collagen and elastin, making the skin feel smoother and tighter. We treat only the area that needs attention. We do not work on any tissue outside the boundaries of the wrinkles and scar tissue as this will make the wrinkle appear even deeper. Skin Needling stimulates the body’s natural wound healing reaction to produce natural collagen.

With microscopic punctures in the epidermis, Skin Needling is minimally invasive with little damage to the epidermis. Anesthesia is generally not needed for Skin Needling as it is not so invasive as to cause pain. Who Will Benefit from Skin Needling?

• In view of the fact that Skin Needling is a nominally invasive procedure, anyone with fine, medium or heavy wrinkling will experience positive results:

• People with acne scars, chicken pox scars, and scars from surgery

• People who have picked at pimples and scabs creating scars and pits

• People with naso-labial wrinkles as well as smokers and laugh lines

• People with chest, neck, knee, arm and hand wrinkles

Contraindications for Skin Needling

• Anyone who is darker skinned and hyper-pigments will need to use a lightening skin treatment for 2 weeks prior to their treatment appointments

• If you have the propensity to keloid you would not be a candidate for Skin Needling

• We never work on raised moles, warts or any raised lesions

People who are not acceptable candidates for this procedure are:

• Anyone that is under medical care that may be on a high blood pressure medication, heart medication, is diabetic, not in general good health, is a poor healer, has an active herpetic breakout, or has just had an invasive laser treatment

• Anyone that has any type of facial skin disease or disorder that would be a contraindication

• Anyone who is taking homeopathic, vitamins or herbs that thin the blood

• Anyone with scars that are less than 6 months old

• Anyone that has had facial fillers, or Botox, in the past 4 months in their lips or face

• Anyone who has had facial surgery in the past 6 months

• Anyone who has had ‘PERMANENT’ fillers, such as Dermagen or Shark Cartilage injected in the past 6 months would not be a candidate for Skin Needling

• Anyone that has been using corto-steroids on the treatment area, as this thins the skin’s tissue

Tools Used

Skin Needling can be accomplished by using a coil, digital, rotary machine or a manual tool. Although this procedure does not require the power of a heavy duty machine, if you are accustomed to using a coil or digital machine you can turn the power down and achieve successful results. Sterile, single-use needle clusters will depend on the method of procedure, depth of area to be worked on, and condition of the tissue.

Pre-Treatment Management

The skin should be assessed prior to the client’s first treatment. The use of skin treatments and/or products both pre- and post-treatment will enhance the client’s results dramatically. Sun worshipers and men that have not been on a regular skin care regime would receive greater benefits if they have any of the treatments listed below prior to their Skin Needling procedure.

Several options are available to the clients Pre-Treatment are:

• Microdermabrasion

• Peels – AHA’s, Glycolic, Salicylic, Malic, Enzyme Peels or Jesners

• Laser

• Retin A, Retinol, Renova

Anyone that is prone to herpetic outbreaks needs to be on an antiviral medication prior to their Skin Needling procedure. This applies to each additional procedure as well. For clients that bruise easily, it is advised to consider taking Arnica Montana, Bromaline, or any other anti-inflammatory and anti-bruising product 1-2 days prior to each Skin Needling session and 2-3 days post procedure.

Post Treatment Management

Mineral makeup can be used after the skin has healed. If you use makeup on broken skin, the risk of bacteria entering the procedure area, as well as the makeup filtering its way into the open tissue, can be detrimental by causing darker pigmentation.

After the skin has healed, the use of AHA’s will help to keep the top layer of skin softened and sloughed off resulting in younger looking skin. For the first few days our clients use a very light coat of Petroleum directly on the treatment area. This will not only keep the area from scabbing but will keep the skin soft and supple.Using topical Vitamin C, A or E and Collagen producing products are also fantastic for the skin. I have been using Prevage by Allergan (from my Dermatologist Dr. Noushin Parastouk) as it is great for sloughing off and regenerating tissue.

Turning Back the Hands of Time

Our clients report smoother, softer feeling and looking skin after each treatment; noticing creams and lotions penetrate their skin more easily, and their fine lines and wrinkles have softened considerably. Clients with other concerns, such as acne scaring, scars from surgery or injury, do not notice the depressed area. The body’s natural collagen is masking the imperfection of the scarring process after each treatment, while it regenerates the natural collagen. We have clients that have seen immediate, long-lasting results, and other clients noticed an improvement gradually over an 8 week period. One of our acne clients said he noticed results 3 months post procedure and his skin kept looking better and better.

Post Procedure Appearance includes:

Day 1 and 2 – Depending on how deeply the technician inserts the needle into the

epidermis, the tissue may be slightly to moderately swollen, tender, red and bruised,

with a slight lymph discharge from the treated areas. Minor itching may occur and the ‘needled’ tissue may exhibit the appearance of ‘cat scratches’.

Day 3 – The treated areas slightly crust and remain faintly pink to red.

Day 4-–The redness and crusting have diminished.

Day 5—There is barely any evidence of a procedure.

Follow up Treatments

Procedure applications are usually repeated at a 6-8 week intervals depending on the condition of the tissue and desired results.

Post Procedure Aftercare for Skin Needling

Skin plumping products, vitamins or collagen based products can be used after the skin is healed. The use of a ‘Total Waterproof Sun Block’ is strongly suggested on a daily basis.

Facial Fillers

After the Skin Needling procedure fills in wrinkles and lines with the body’s own natural collagen, the client may still want to have facial fillers in their lips, as well as Botox, to paralyze their glabella, upper forehead and crow’s feet.

Final Thoughts

To look their best, we still recommend that all clients should consider having Botox to paralyze their muscles to enhance their Skin Needling results. Lip fillers are also highly recommended to plump the lips if the client is requesting fuller, luscious lips. Skin Needling can be dangerous in the untrained hand. Skin could be macerated by overworking of the procedural area and potentially creating scarring. Always consult a trained professional for these treatments. People trying to do these treatments at home will not experience pleasing results.
